Fix links (http -> https, updated locations)

This MR changes all instances of http:// to https:// in the code and the docs (except for the links to CPS in the README, see issue #35). In doing this process, anywhere where I found a broken link I fixed it. This does not fix all potentially-broken links — reviewing those is a much larger task.
