Update ceus grid pdfs - add decluster branch

  • New spatial PDFs for GK, NN, and R85 declustering methods
  • Add gk, nn, r85 source group branches (equally weighted)
  • Arbitrarily updated rupture sets with temporary IDs (append 1 for gk, 2 for nn, 3 for r85)
  • Retained existing fixed/adaptive total rates in rate-tree.json, pending new values

Note: new grids do not appear to have the same smoothing extent as the 2018 grids (i.e. adaptive PDF appears to be limited to the extent of the CONUS map region used for calculations), see comment on issue #25 (closed)
